About our Wellness Program

Disease Management

Disease management is a system of coordinated healthcare interventions and communications for populations having conditions in which patient self-care efforts are significant.   

Rather than targeting individual disease symptoms, the MedWatch Disease Management program uses a comprehensive, integrated approach for the whole person.  Most participants have several co-morbid conditions including diabetes, cardiovascular, chronic obstructive pulmonary disease (COPD)/asthma, chronic pain, arthritis, digestive disorders, and obesity.  The MedWatch Disease Management program is participant centered and designed to:

  • Promote Patient Self-Care
  • Provide needed participant teaching and support
  • Improve participant compliance to treatment plan
  • Educate to reinforce healthy lifestyle behaviors
  • Avoid complications
  • Reduce claim costs
  • Decrease hospital days and ER visits
  • Ensure continuity of care

The MedWatch Disease Management Program identifies potential participants through a variety of means including:

  • History of inpatient confinements from MedWatch Utilization Review
  • Employer health screenings
  • Review of Health Risk Assessment
  • Review of trigger diagnosis reporting
  • Review of claims data
  • Review of pharmacy data
  • Review of disability claims
  • Review of lab data
  • Referral from MedWatch case management

The key to success is the relationship the Total Lifestyle Counselor develops with the participant.  The Total Lifestyle Counselor contacts the participant initially to determine what the participant understands about their medical conditions and treatment plan.  With the participant, the Total Lifestyle Counselor develops an individualized plan tailored to the individual learning needs and learning style of the participant as well as their level of motivation and stage of behavior change.   During contacts with the participant, the Total Lifestyle Counselor provides education on the medical conditions, medications, and treatment plan as well as education to reinforce healthy lifestyle behaviors including: smoking cessation, weight management, stress management, exercise, healthy eating, etc.  Education includes verbal information, internet resources, and printed educational materials.

The MedWatch Disease Management Program provides outcome reporting for disease specific outcome measures (i.e. blood pressure, cholesterol, blood sugar values, A1C, etc.).  Quarterly reports are provided for each participant that includes a summary of activities as well as medical status updates.  MedWatch also provides quarterly aggregate reports which include summaries of activities, contacts, compliance, improvements, etc.
